We have a Joomla website that uses a lightly modified version of ExtendedReg (from JVitals). My modifications to ExtendedReg are small, but done in the core files of the extension. I only added a new status for the user, and some e-mail functions, and prevented some other mail functions to happen... (Budget: $30-$250 USD, Jobs: Joomla, PHP, Software Architecture)
